On November 19th, the National Health Commission (NHC) of China held a training session in Sanming, Fujian Province, to promote the Sanming healthcare reform experience. Lei Haichao, Party Secretary and Director of the NHC, emphasized that provinces should utilize cities as units and take concrete actions to earnestly promote the Sanming experience, achieving comprehensive and dynamic progress in reform. This meeting marks a new phase in the promotion of the Sanming experience and highlights its significant position and exemplary role in China's national healthcare reform. The Sanming reform, particularly its 3.0 phase innovationsthe upgrade from "Medical Community of Practice" (MeCoP) to "Healthy Community of Practice" (HeCoP) and the establishment of the "Six-Disease Collaborative Management" modelprovides valuable experience and lessons for national reform efforts.
The launch of Sanming's healthcare reform 3.0 signifies a new stage. A core objective of this phase is the upgrade from MeCoP to HeCoP. HeCoP, compared to MeCoP, places greater emphasis on health management and prevention, aiming for whole-life cycle health management and a true shift from "treating illness" to "preventing illness." The "Six-Disease Collaborative Management" model serves as a crucial tool for HeCoP construction. It leverages digital technology to build a four-tiered digital management system ("city-county-township-village"), enabling the "horizontal transfer" of advanced diagnostic techniques to primary-level facilities.
The "Six-Disease Collaborative Management" model is not merely disease management but encompasses a whole-life cycle approach to disease prevention, screening, diagnosis, treatment, management, and rehabilitation. Its core lies in multidisciplinary collaboration, breaking down information barriers, and forming a patient-centered, whole-life cycle health management framework. Diagnosed with right lower lobe small cell lung cancer, she received treatment and was able to "remotely" consult with top experts from Shanghai Ruijin Hospital, benefiting from comprehensive, multidisciplinary, whole-life cycle care for her tumor. Via video conferencing, a multidisciplinary team (MDT) comprising specialists from different departments developed a detailed treatment plan.
Ms. Zhang's case is not an isolated incident; it represents the significant achievements of Sanming reform 3.0 and highlights the importance of telehealth in primary care. This model significantly improves the diagnostic and treatment capabilities of primary hospitals, allowing patients access to higher-quality medical services while avoiding the inconvenience of traveling between different hospitals and departments, truly embodying the concept of "easy access to healthcare."
The collaborative model of "Ruijin Medicine + Sanming Demonstration + WeDoctor AI" is key to the successful implementation of the "Six-Disease Collaborative Management" model. Shanghai Ruijin Hospital's established "Six-Disease Collaborative Management" standards and techniques are precisely replicated and transferred to Sanming using digital technologies. This success relies on technological platform support, medical professionals' expertise, and the importance of cross-regional medical collaboration.
Currently, the initial phase of the "Six-Disease Collaborative Management" model has completed the construction of collaborative management service systems for cancer and metabolic diseases. Systems for cardiovascular and cerebrovascular diseases, respiratory diseases, reproductive medicine, and geriatrics will be implemented successively. Sanming has also established a "Six-Disease Collaborative Management" centralized procurement center, promoting disease-specific centralized procurement to provide patients with more cost-effective diagnostic and treatment options and optimize the use of medical insurance funds.
